This is a verified petition filed by petitioner, NEMIA TAYOTO, through counsel, praying that after due notice, publication and hearing, a judgment be rendered ordering the Local Civil Registrar of Baras, Catanduanes and the Civil Registrar General to CORRECT the entry in the Certificate of Live Birth under Registry No. 98-98 by changing petitioner’s surname from CARRANZA to TAYOTO.

Finding the petition to be sufficient in form and substance, this Court hereby sets for initial hearing the aforementioned petition on February 20, 2018 at 8:30 o’clock in the morning, before this Court sitting at the Provincial Capitol, Virac, Catanduanes, and directs that a copy of this order be published at the expense of the petitioner in a newspaper of general circulation in this province, once a week for three (3) consecutive weeks. All persons opposed to the petition may file their opposition on or before the date of hearing before this Court, to show cause, if any, why the aforesaid petition shall not be granted.

This is a verified petition filed by petitioner, JULIUS RONALD SAN JUAN SORRA, through counsel, praying that after due notice, publication and hearing, judgment be rendered:

A, Declaring the petitioner not the father of the child named Cherlind Franchette Sarmiento Sorra;

b. Declaring the status of Cherlind Franchette Sarmiento Sorra as an “illegitimate” child;

c. Ordering the Local Civil Registrar of Muntinlupa City, and the Philippine Statistics Authority to CANCEL, DELETE the name – JULIUS RONALD SAN JUAN SORRA as the name of the father in the birth record of Cherlind Franchette Sarmiento Sorra;

Finding the petition to be sufficient in form and in substance, this Court hereby sets for initial hearing the aforementioned petition  on December 13, 2017 at 8:30 o’clock in the morning, before this Court sitting at the Provincial Capitol, Virac, Catanduanes, and directs that a copy of this order be published at the expense of the petitioner in a newspaper of general circulation in the province, once a week for three (3) consecutive weeks. All persons opposed to the petition may file their opposition on or before the date of hearing before the Court, to show cause, if any, why the aforesaid petition should not be granted.

Let a copy of this Order together with the petition be furnished the Office of the Local Civil Registrar of Muntinlupa City and the Office of the Philippine Statistics Authority.

A verified petition was filed by the petitioner, through counsel, praying that after due notice, publication and hearing, a judgment be rendered ordering the Local Civil Registrar of Caramoran, Catanduanes and the Civil Registrar General to correct the entry in the Birth Record/Certificate of herein petitioner, Romeo de la Rosa Palero, in his Birth Date entry from October 7, 1959 to October 7, 1957.

Finding the petition sufficient in form and substance, let this Petition be set for initial hearing on September 1, 2017 at 6:30 o’clock in the morning at which time and date, any interested person may appear and show cause, if any, why the petition should not be granted.

Let this Order be published at petitioner’s expense once a week for three (3) consecutive weeks in a newspaper of general circulation in the Province of Catanduanes and the copy thereof be furnished the Office of the Local Civil Registrar of Caramoran, Catanduanes, the Philippine Statistics Authority, Quezon City, and the Office of the Solicitor General. The Branch Clerk of Court is ordered to furnish the Office of the Clerk of Court of this Order.
