This is a verified petition filed by petitioner, NEMIA TAYOTO, through counsel, praying that after due notice, publication and hearing, a judgment be rendered ordering the Local Civil Registrar of Baras, Catanduanes and the Civil Registrar General to CORRECT the entry in the Certificate of Live Birth under Registry No. 98-98 by changing petitioner’s surname from CARRANZA to TAYOTO.

Finding the petition to be sufficient in form and substance, this Court hereby sets for initial hearing the aforementioned petition on February 20, 2018 at 8:30 o’clock in the morning, before this Court sitting at the Provincial Capitol, Virac, Catanduanes, and directs that a copy of this order be published at the expense of the petitioner in a newspaper of general circulation in this province, once a week for three (3) consecutive weeks. All persons opposed to the petition may file their opposition on or before the date of hearing before this Court, to show cause, if any, why the aforesaid petition shall not be granted.

Let a copy of this Order together with the petition be furnished the Office of the Local Civil Registrar of Baras, Catanduanes, and the Office of the Civil Registrar-General, Manila.

All persons opposed to the petition may file their opposition on or before the date of hearing before the Court, to show cause, if any, why the aforesaid petition should not be granted.
